Step 1: To create your survey, please click here and enter your telephone number in the space provided to sample the outbound capability of the Telvista Automated Agent or call 1-800-581-7906.

Step 2: Please, follow the prompts to create your own survey. Click here for a sample survey script or feel free to make up your own questions. **Be sure to stay on the line at the end of the call for important steps on how to access your survey and view an online report.**

Step 3: The Telvista Automated Agent will create an Access Code on the phone. Be prepared to write it down as you will need it in order to access your survey. You survey will be available for 24 hours.

Step 4: Hang up and dial 1-800-724-7968 to access your survey.

Step 5: Enter your Access Code when prompted by The Telvista Automated Agent.

Step 6: Respond to your survey questions.

Step 7: Click on view reports to see your survey details. You will be asked to enter your Access Code. Note: Clicking on the question number in your report will replay your survey question using a standard media player.
